Due to some Roadblocks Yvonne and I ran into with the previous Partner Application form, He lead us to create a simpler form.
Due to the complexities of the previous form, we ran into revision issues and couldn't edit it any further, no matter if we tried to create a new one or to export it to other websites.
So please have a look at the simpler form and please let us know if we must change anything.
